This is where you can find everything you want to know about long-term care services in Chittenden County.

WHO IS IT FOR?  Individuals who need long term care and their families, and medical care and social service providers.

WHAT'S IN IT?  You can find information about housing, care at home, transportation, Medicaid, nursing homes, Social Security, and more. You can link to state and federal government sites.

DISABILITIES  Although most of the information on this page is useful for people of any age who need long term care, many of the services listed are related to the needs of people 60 years old and older.  For more specific information about services for younger people with disabilities, look for the sign:

This will direct you to the Vermont Center for Independent Living, vcil@vcil.org, which specializes in information, referral, and peer support for younger people with disabilities.

WHO PUT THIS PAGE TOGETHER?  The Champlain Long Term Care Coalition, a group of Chittenden County agencies and long term care consumers, saw the need for a comprehensive and frequently updated guide to services.  Fletcher Allen Health Care agreed to host and maintain this site on their server.  Champlain Valley Agency on Aging has started it off with information from their written resource guide and will provide monthly updates.
