Fletcher Allen Health Care and The Center for Elder Health (CEH) are eager to work with community partners and consumers to develop a system of services to allow older adults to stay healthy and live in the setting of their choice for as long as possible.
To this end, CEH is involved in several community focused initiatives:

Champlain Long Term Care Coaltion (CLTCC)
Information and strategic plan for CLTCC
The CLTCC is made up of members of the community, providers and advocates for long term care who are interested in forwarding the strategic direction of long term care in Chittenden and Southern Grand Isle counties.
The Champlain Long Term Care Coalition has gathered data as part of the Community Partnership for Older Adults development grant from the Robert Wood Johnson Foundation. Vermont’s CLTCC was one of only 12 partnerships in the United States to receive this coveted funding.

The Falls Program
A simple fall can have a severe impact on seniors. Often it results in a fractured hip, the most common injury to seniors. It usually leads to a long and expensive hospital stay, surgery, potential complications, and lengthy recuperation, often involving long-term care. Treatment costs and lost income averages $35,000 for hip fractures. Half of all elders who break a hip die within a year.

Housing and Supportive Services Programs (HASS)
A collaboration between:

Burlington Housing Authority
Grant funded, State of Vermont, Department of Aging and Independent Living
The HASS Program
Founded in July 1998
Funded by the Department of Aging and Disabilities through the Housing and Supportive Services Program or HASS. The two HASS sites in Burlington are:

Currently HASS at Decker Towers serves over 180 residents. Participation is voluntary but open to all residents
Goal: to allow older adults and younger adults with disabilities to age in place. Outcomes measures include nursing home admission, incidence of falls and hospitalizations, use of services and number of residents assessed.

Supervision is by Jeanne Hutchins at Elder Care Services, Fletcher Allen Health Care.

SERVICES:
- Overall risk assessment using the Independent Living Assessment with emphasis on identifying risk factors for nursing home placement.
- Referrals are made to appropriate service agencies and coordinated with case managers (CM) from other agencies. Shelly defers to agency CM as lead CM as our role is to be available for those who don't have CM or services in place or to take over when agency discharges patient from their services.
- The HASS coordinator provides case management (CM) as needed.
- Funds are available to purchase services that might otherwise not be provided.
- For some residents, the assessment is purely a baseline and a preventive/educative service since they are very high functioning and need no services to remain independent. Others need both community and in house services to stay in their home.
- Provides individual and group self care and chronic disease management as well as nursing home risk reduction education and services. Does BPs, glucose screenings, footcare, and exercise class weekly and as needed.
- Provides nursing services as needed for those who cannot be served by other agencies. Will do some short-term interventions to bridge the gap between services on a case by case basis. Available for followup of residents with active problems or who may have been seen in the ED or MD office and require further assessment
- Triage for questions and problems as they arise.
