Outreach

Adult Outreach	A variety of adult services are offered throughout the community by Community Health Improvement including: Diabetes Education and Fitness, Falls and Fires (home safety), Footnoters, Lifeline, and community screenings.  To learn more about these programs, visit our web page or contact Pam Farnham, RN, Adult Outreach Coordinator, at 847-2394.
Pediatric Outreach	We partner with health care providers, schools and local community groups to support community wellness and disease management programs. Our services respond to community health priorities and needs such as injury prevention, car seat safety, hearing outreach, poison prevention education, dental access, and more.
Health Assistance	Fletcher Allen Health Care offers low-cost or free access to health care for individuals who cannot otherwise afford services. For more information visit our web page or contact Jennifer Daley, Health Assistance Coordinator, at 847-6985.
