Comprehensive Care Clinic

The Comprehensive Care Clinic is the Vermont state referral center for HIV/AIDS. The physicians, nurse practitioner, nurse, and social worker of the clinic work as a team for the patient's medical, psychosocial, and economic health issues.  The clinic works closely with primary care physicians, state agencies, and regional AIDS service organizations to provide state-of-the-art care for people with HIV/AIDS.  The Comprehensive Care Clinic telephone number is 802-847-4594.

The Comprehensive Care Clinics (CCC) are four statewide HIV clinics serving the entire state of Vermont. The Burlington CCC was established in 1987. The Rutland, Brattleboro and St. Johnsbury CCC were established in 8/94, 8/95 and 10/96 respectively, with support from the Special Projects of National Significance (SPNS) section of the Health Resources Services Administration (HRSA). In 1999 funding shifted to Title III of HRSA. Barriers to providing a continuum of care for persons with HIV infection in rural Vermont including long travel distances to receive state of the art health care, long harsh winters, limited public transportation, concern about confidentiality in small towns, and limited HIV expertise among regional providers have been overcome. The program is currently in its 10th successful year. Last year, we provided state-of-the-art HIV care and case management to 310 active patients and 60 new patients.

The CCC provides primary and infectious disease specialty care for people with HIV disease. Each clinic is housed in a regional hospital or hospital outpatient clinic. The Burlington clinic, which cares for the majority of patients, is staffed by a nurse practitioner and nurse, three infectious disease physicians, a social worker, dietician and support staff. The three other clinics are similarly staffed except that an infectious disease physician travels to those clinics once per month.

The clinics provide complete medical care for people with HIV at all stages of disease. Screening, immunizations, anti-retroviral therapy, prophylaxis, diagnosis, referrals and treatment of opportunistic infections and ancers are provided. Current federal guidelines concerning antiretroviral therapy are followed. Seamless inpatient to outpatient care is coordinated by the physicians and nurse practitioners. Patients are evaluated for drug and alcohol abuse and psychiatric illness and referrals are made for assessment and management. Clinic team members are active in hospice care and family support. All clinics are Vermont Department of Health designated anonymous testing sites.

International Travel Medicine

An international travel medicine service opened in 1996 at the UHC Campus of Fletcher Allen Health Care to prepare area travelers for their trips out of the country.  The service provides counseling and vaccines, based on the traveler's destination(s), as well as a country-specific printout that includes information about entry requirements, areas of instability, medical facilities, information on crime, along with general and specific health precautions.

The International Travel Medicine service now located on the Medical Center Campus, is a designated center to administer the yellow fever vaccine.  Patients can call the Travel Health Service at 802-847-1045 or toll-free at
1-800-358-1144.  Providers should call the Provider Access Service at 1-800-639-2480 and ask for the Infectious Disease physician on call.

If the patient is a member of a managed care plan, a referral is needed. Patients will be sent a questionnaire prior to their visit.

Referral Information

Patients may self-refer to the HIV Service and the International Travel Medicine service.  All other conditions require a physician referral.
