Retina SpecialistsDear Patient: You will be evaluated in the Retina office by one of us. We are each Fellowship-trained Retina and Vitreous specialists. We are eager to work with you to provide the best care possible for your eye condition. We will do everything in our power to make your visit as comfortable and informative as possible. We will perform a new patient consultation or visit when you are seen. Before you actually see us, you will be checked-in and evaluated (history discussed, vision checked, eye drops placed, etc.) by our secretaries and technical staff. You may require further testing including photographic tests termed fluorescein angiography or indocyanine green angiography. Occasionally, a computerized test of your central and side vision called a visual field may need to be done. If you have age-related macular degeneration, a retinal hole or detachment, or diabetic retinopathy, you may need to have laser done. Dependent upon the urgency of the situation, these additional tests and/or treatments may need to be done that day or may be scheduled for another day. Suffice it to say that the visit may take several hours and, in fact, if you require additional testing and/or laser, you could conceivably be in the clinic for much of the day. Please try to maintain flexibility of scheduling on this day. It would probably be appropriate to avoid scheduling other appointments on this day, unless it is absolutely necessary. We look forward to seeing you. Sincerely,
Please print out the "Medical Questionnaire" form below and fill it out before arriving at our office. This will help to expedite the check-in process. |
