Routine Consultations
When submitting slide consultations to Pathology, please include the following:
- A copy of your report
- Patient billing information
- Full name (no nicknames)
- Date of birth
- Social Security Number
- Address
- Insurance information
- Parent/guardian information on children
Cover letter should include:
- History
- Specific questions/issues to be addressed
- Other than the submitting pathologist, the name and address of other physicians who should receive a copy of our report.
- Slides
- Blocks
If there is a possibility that we will need to do special stains, levels, or H+E recuts, please include at least one representative block.
- X-rays
Please include copies of pertinent x-rays for all bone lesions.
